Introduction: the epidemiology of uterine leiomyomas

Uterine fibroids are the most common tumors in women during the reproductive years, and are the most frequent indication for hysterectomy in most countries in the world. This paper discusses the socio-economic and social impact of these tumors. (US fibroid-related costs in 1990 were $3 billion.)

Predictors of subsequent surgery for uterine leiomyomata after abdominal myomectomy

In 65 women who had abdominal myomectomy by the same surgeon, there was an increased risk of subsequently needing a second surgery if the uterus was under 12 week size at original surgery.

Complementary medicine treatment of uterine fibroids: a pilot study

Matched case control study of women with symptomatic fibroids treated with acupuncture, Chinese herbs, nutritional therapy, pelvic bodywork (myofascial release and deep tissue massage), meditation and guided imagery vs. conventional therapy with hormonal manipulation and non-steroidal anti-inflammatory medication. Over six months, fibroids disappeared, shrank or stayed stable in 22/37 of the treatment group vs. 3/37 in the control group. However, both groups had an equal decrease in symptoms of abnormal bleeding and cramping. Costs for the treatment group were $3800/patient.

Etiology and treatment of uterine fibroids
Comment on previous study, delineating the difficulties of providing such a comprehensive array of complementary therapies, the time and costs involved, practitioner knowledge base, potential problems with herbal medicine, etc--especially in view of the fact that there was no difference in symptoms of bleeding or cramping between the two groups.  Author calls for future well designed studies combining standard and complementary therapies.

Influence of exogenous estrogen receptor ligands on uterine leiomyoma: evidence from an in vitro/in vivo animal model for uterine fibroids

Uterine leiomyomata show an enhanced sensitivity to estrogen. Organochlorine pesticides, dietary flavanoids, botanical extracts, and therapeutic antiestrogens can influence fibroid growth.

Different expression of estrogen receptors alpha and beta in human myometrium and leiomyoma during the proliferative phase of the menstrual cycle and after GnRHa treatment

Both alpha and beta estrogen receptors were analyzed in fibroid as well as normal uterine myometrial tissue. Alpha receptors were higher in the fibroid than the normal tissue. After treatment with GnRHa, the alpha receptors were higher, and the betas lower than normal tissue, leading to an increased alpha/beta ratio. This increased ratio could reflect or be the cause of the shrinkage of the fibroid.

Progesterone: a critical role in the pathogenesis of uterine myomas

The transformation of myometrium to fibroid involves mutation of normal uterine muscle cells and a complex interaction of sex steroids and local growth factors. In addition to estrogen, progesterone plays a role in growth.

Pathology of uterine fibroids

Review of the pathological appearance of uterine fibroids on a macroscopic, histologic, and ultrastructural level. Review also of the effects of GnRH agents on fibroids, with "considerable variation in the pattern of response from case to case and maybe even from tumour to tumour." Results ranged from no change to necrosis.
